The Qpp Xtensions are used to control QuarkXPress as Q++ scans, generates or SARAs. There are 2 sets of mutually exclusive options available from this menu's sub-menus :

enabled

disabled

The Qpp Xtensions are installed with Q++ and are usually always enabled. However these Xtensions are disabled in the rare cases when you need to launch the internal Quark. Usually these are automatically re-enabled, but if they are not (QuarkXPress launches but nothing happens) then you can use the Qpp Xtensions menu item from the tools menu to manually re-enable them.

Note that the enabling/disabling of Xtensions, and the selection of the safe or beta version of an xtension, is limited to your workstation and does not affect other users.
